Excerpt: Examination approach

From 18 to 21 March 2024, a team of four inspectors, whom I have authorised to carry out visits to places of detention on my behalf, made an announced four-day inspection as part of my examination of Te Awhina Unit (the Unit) at Whanganui Hospital. 

This examination was to follow up on the 14 recommendations I made in 2021, following my inspection of the Unit in September 2020. The Unit’s management had fully accepted 11 of these recommendations and partially accepted a further three recommendations. 

My inspectors gathered and assessed a range of information, including reviewing documents, interviewing a number of people, and observing facility activities. Before leaving the facility, inspectors met with the Unit’s management to outline their initial observations. Inspectors continued my examination activity post-inspection, with in-depth analysis of information collected on and after the inspection. Inspectors were guided by my expectations for conditions and treatment of tāngata whai ora.
