Required information shown with

Before you start, do you have:

  • Copies of any material you wish to provide about your complaint ready in electronic format? You can post this material to the Ombudsman later if you wish.
  • Written authority to act if you are complaining on behalf of someone else, but you’re not their licenced representative?

If you need help, call 0800 802 602 to discuss.

There are four pages in this form.

What is the nature of your complaint?

Important note: Have you tried to resolve the problem yourself first?

Before you make a complaint to an Ombudsman, you should first try to resolve it with the agency concerned. Many agencies have a complaints process. If this is not the case, you should write to the head or Chief Executive of the agency and make your complaint. An Ombudsman may decide not to investigate your complaint unless you have attempted to do this.

Complaining on behalf of another person?
Are you making a complaint on behalf of another person?

If you’re making a complaint for someone else, you need to be authorised before we can act on the complaint.

If you’re their lawyer, accountant or other licensed professional, you are already authorised.

Otherwise – for example if you’re family or a friend – you’ll have to give us their written authorisation for you to:

  • make a complaint on their behalf
  • receive any personal information about them.
Are you their lawyer, accountant or other licenced professional?
Do you have written authority to act on behalf of this person?

Important note: We may not be able to help if you’re complaining for someone else, and you don’t have their written authority to do so.

Please make sure you get this authority so we can process your complaint. You can complete this form now and send the authority later by email or post.

You need to give us a copy of the authorisation. How are you sending it?
(If sending material by post please label clearly with your personal details)
